Alternative claim and method

If you are unable to use the ServiceWA app to reserve your vouchers and submit your rebate, you can claim via an alternative claim form. Complete the form and upload copies of your identity documents to meet 100 points of ID from the examples provided.

Alternative claim form

Alternative claim forms will be processed within 10 to 15 business days. Alternative claims will receive an email update from the Kids Access All Areas Vouchers program team to their nominated email address, with voucher code confirmation and instructions for how to claim your rebate or relevant status updates.

100 point checklist for identification documents

  • You must supply at least one primary document.
  • Total points score must equal 100 points.
  • Foreign documents must be accompanied by an official translation.
  • All documents require your full name.
  • Evidence of change of name is required (for instance, marriage certificate or deed poll) if the name on any of the documents presented is different, as a link between former name and current name.

Primary documents

The following documents are 70 points:

  • Australian passport (current) — requires photograph
  • Australian driver's licence or learner’s permit — requires address and photograph
  • foreign passport (current) — requires photograph
  • Australian citizenship certificate
  • full birth certificate (not birth certificate extract)
  • Certificate of Identity issued by the Australian Government to refugees and non-Australian citizens for entry to Australia.

Secondary documents

The following documents are 40 points:

  • Department of Veterans Affairs (DVA) card — requires address
  • Centrelink card (with reference number) — requires address.

The following documents are 25 points:

  • birth certificate extract
  • Medicare card
  • credit card or account card
  • Australian marriage certificate (Australian Registry issue only) — requires signature
  • decree nisi or decree absolute (Australian Registry issue only) — requires signature
  • change of name certificate (Australian Registry issue only) — requires signature
  • bank statement (showing transactions) — requires address
  • property lease agreement — current address
  • taxation assessment notice — requires address
  • Australian mortgage documents — current address
  • rating authority — current address (for example, land rates).

The following documents are 20 points:

  • utility bill: electricity, gas, telephone — current address (less than 12 months old)
  • reference from an Indigenous organisation — requires photograph
  • documents issued outside Australia (equivalent to Australian documents) — must have official translation attached and requires photograph.

About WAIVPAY

Waivpay Limited is a third-party payment provider used by the WA Government to process payments and reimbursements for certain programs, including the Kids Access All Areas Vouchers program.

Although the Kids Access All Areas Vouchers program is run by the WA Government, payment processing is managed and facilitated by WAIVPAY, a trusted third-party provider that operates nationally. Claimants who reserved their voucher via the alternative claim process will need to use the WAIVPAY portal when making their rebate claim.
