Photo, video and audio consent form

The Department of Creative Industries, Tourism and Sport does not use photographs, videos or audio of members of the public without their prior written consent.  

Where a person is under 18 years of age, we require the consent of a parent/carer.

We will take reasonable steps to ensure the photo, video and audio is used solely in accordance with your consent provided below.

In providing my consent, I acknowledge that:

  1. I confirm I have read or been made aware of how my/my child’s images, video, and/or audio will be used (as indicated above)
  2. if I am signing on behalf of a child, I am that child’s parent or legal guardian
  3. my permission can be withdrawn at any time by written request to and confirmation from the department through commsapprovals@cits.wa.gov.au
  4. I am not entitled to any compensation or payment for the taking or use of the materials 
  5. the department will not sell the materials or share the image with any third party, other than outlined above
  6. where photos, videos, audio are required to be used otherwise than as outlined in this consent form, the department will take reasonable steps to provide a new consent form to gain further permission
  7. prior to signing this consent form, I may in writing, request a copy of the department’s privacy policy.
